To Apply for Head Start:
1. Complete the “Getting to Know You” form and bring it with you on your appointment date.
2. The Family Service Worker will determine if your child & family meet the criteria for acceptance. The process may take 1-2 hours.
3. You will be notified when your child has been accepted and when and where the other screenings, meeting, orientations and home visits will be scheduled.
Required Documentation
1. When applying, you must be able to show proof of income. This may include 2 pay stubs, W-2’s, Tax Returns, or a statement from your employer.
2. In addition, we will need to see your child’s Birth Certificate, Social Security Card, and Immunization record.
3. Proof of residence may be, Electric Bill, Phone Bill or Gas Bill.
4. We can not consider any application until Head Start Family Service’s staff have reviewed this information.
